We love to work with other healthcare providers to give our patients optimal care. When you refer a patient to us, rest assured that they will receive thorough diagnosis, customized care and personal communication with our doctors and staff. We will keep your doctors and practitioners updated on your patient’s treatment and progress as they work with us.

We’ve provided a simple referral form for you to download, fill out and fax to us at 952/832-0766. This will allow us to set up the initial visit for your patient as promptly as possible. Appointments are usually available within one to three days of your referral.
